WAIVER, RELEASE OF LIABILITY AND PERMISSION TO USE LIKENESS

I agree and represent that my participation in the Donate Life Legacy Walk at the National WWI Museum and Memorial on June 3, 2023, entails known and unknown risks that could result in serious bodily injury or illness and that I am qualified and in proper physical condition to participate. My participation in the event is completely voluntary, and I knowingly and freely accept and assume all risks, losses, illnesses, costs and damages associated with my participation in the event. I, the undersigned person, voluntarily release, forever discharge and agree not to sue Midwest Transplant Network, its parent company, affiliates, administrators, directors, officers, volunteers and employees (Released Parties) from all liability, claims, demands, losses or damages on my account caused or alleged to be caused in whole or in part by the action or inaction of the Released Parties, including negligent rescue operations.

I understand I will be required to comply with all Midwest Transplant Network Donate Life Legacy Walk COVID-19 protocols/precautions of which I am informed if I chose to participate in person.

As a participant in the Donate Life Legacy Walk, I hereby release and allow Midwest Transplant Network the right and permission to use in perpetuity my image, likeness, voice and appearance in any manner and in any media now known or later developed for the purposes of advertising and publicizing Midwest Transplant Network’s works and mission.
